Supported Living
Our Supported living services are designed to help people live happy lives filled with ambition. Whether it’s learning a new life skill such as cooking or money management, having the confidence to take up a new hobby or make new friends, or taking the first step into volunteering or employment, we’re here to support and encourage each goal, however big or small.
We run small homes for just 1, 2, 3 or 4 service users, with high staffing ratios to provide the care and support required.
Our support plans are person centred with built clear understanding of each service user history, their current situation, and their future goals. We have in-depth conversations with the person, their family, and any relevant care professionals to make sure that we have all the necessary information and have taken the time to hear each person’s views and wishes before we provide our care. Once a supported living support plan is in place, we regularly review progress, recognising and celebrating every achievement and continuing to encourage each person to set new goals to achieve even more.
Residential Care
All service users living in our residential care accommodations have their own unique needs and aspirations. Meaning every single home is unique too. We offer our services 24 hours a day, 7 days a week, dedicating ourselves to helping each individual to lead a rich and fulfilling life.
Each home is designed carefully to keep the people we support safe and comfortable whilst helping them to build new skills, and so many of our homes contain adaptations such as sensory rooms. Some of our homes also have the use of assistive technology where possible which further aids independent living.
Our homes are located in city and rural settings, all set within the heart of communities meaning that they are close to local amenities and have excellent transport links. Each home complies with the regulatory requirements of the care commissions for England, Scotland and Wales.
Domiciliary Care
Our trained and professionals support our service users live independently in their own homes through daily visits or live-in care. Our care allows individuals to remain in their homes and continue living life on their own terms, safe in the knowledge that they have private carers providing all the care and support they require. We support with personal care, assisting with household tasks, medication, and other activities that help to maintain quality of life.
Live- in Care:
We offer a 24- hour service Live-in care that provides full-time care for individuals in their own homes.
Tailored Care:
We understand that some of our service users prefer to do some activities independently and need support with other needs. In JPK Healthcare Homes we offer packages from as little as half an hour a day, up to several visits a day and also overnight care. All our packages are tailored to individual needs.
Mental Health
Our dedicated support provides the very best support for individuals affected by mental ill health, enabling people to live with greater independence and control over their lives.
Our service provides specialist support and recovery services for people with enduring mental health needs, supporting them in the transition from institutional environments back into their own home within the community. With our support, we are able to help people take an earlier transition out of secure units or hospitals than they would have achieved on their own.
Learning Disabilities
Our inhouse care team offer a range of different support for people living with learning disabilities that are designed to meet individual needs. This might help with maintaining independence or providing encouragement to achieve a particular goal. The level of support we provide is completely flexible – whether it’s just a few hours a week or 24-7, we adapt our plan for each person.
Acquired Brain Injury
Each person’s condition is unique, so we ensure their support plan is too. Our dedicated and specialist teams are highly experienced in providing brain injury rehabilitation and community supported living. With our in-depth understanding of acquired brain injuries, we carefully tailor our support to meet the needs of each individual based on their type of injury and throughout all stages of their recovery journey.
Through our community teams, we can arrange physiotherapy, speech and language therapy and occupational therapy, helping to regain everyday skills such as washing and dressing. For those with milder conditions, further along in their recovery or simply seeking to build their independence, we put the focus on maintaining quality of life and opening up opportunities in the community; supporting with everyday tasks such as using public transport, shopping trips and also offering advice and help with starting volunteer or work placements.
Complex care needs
Some of our services users have two or more diagnosis requiring ongoing essential specialist support to manage their symptoms and daily tasks. Examples of such diagnosis include neurological disorders, disabilities, and complex health needs such as a heart condition, epilepsy, or diabetes.
Our highly trained support workers and specialist teams employ a variety of trusted methods including Positive Behaviour Support (PBS) which employs person-centred approaches to ensure a positive impact on each person’s quality of life – supporting people effectively with their needs and understanding the impact that a person’s condition or disease has on their life.
